Traumatic Brain Injury (TBI) Waiver Service Coordination
A qualified care manager coordinates and monitors the provision of all services in the Service Plan including home care, medical services, and transportation. TBI waiver participants must be eligible for Medicaid, have a TBI, and between 18-64 years old.
Nursing Home Transition and Diversion (NHTD) Waiver Service Coordination
A qualified care manager coordinates and monitors the provision of all services in the Service Plan including home care, medical services, and transportation. NHTD waiver participants must be eligible for Medicaid, have a TBI, and between 18-64 years old with a physical disability, or over the age of 65 and in need of skilled care to remain living independently in the community.
Private Pay Service Coordination
A qualified care manager coordinates and monitors the provision of all assistive services and supports including but not limited to home care, medical services, and transportation. Services are tailored to meet your needs. Prices range from $200-$750 per month.
