Care Managers or Service Coordinators use a person-centered approach to connect a person's social supports with health care to facilitate continued health, safety, and community independence. Our qualified team provides better organization of adaptive, medical, and behavioral health needs. It is our goal to ensure a person has access to the network of supports necessary to live safely and independently in their community. Our staff are here to help you obtain and maintain eligibility for your chosen program, secure and maintain housing, medical providers, home care services, transportation, adaptive equipment and home modification, plan for the future, and more.
